Responsibilities Provide direct care to adult and geriatric
patients in an outpatient setting. Collaborate with a
multidisciplinary team to create and implement individualized
treatment plans. Conduct comprehensive assessments and manage
patient care plans. Qualifications Board-certified or
board-eligible in Geriatrics. Valid medical license in the State of
New York. Strong interpersonal and communication skills. Working
Hours Monday–Friday, typical day shift hours. Skills Expertise in
geriatrics and passionate about caring for the elderly. Proficient
in using electronic medical records (EMR). Ability to work
collaboratively within a team-oriented environment.
