Registered Nurse (RN) | Home Health Location: Auburn, NY Agency: Cynet Health Pay: $2,156 per week Shift Information: Days - 5 days Contract Duration: 260 Weeks Start Date: ASAP About the Position Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 days Shift: 5x5 Days Hours per Shift: Not specified Experience: Two (2) years of experience in a home setting within the last ten (10) years License: Current RN Licensure in New York State Certifications: BLS required before the start date Must-Have: - All nurse assessors must meet the following requirements: - Two (2) years of experience in a home setting within the last ten (10) years. - Experience working for a certified home health agency, licensed home care service agency, or managed long-term care. - Experience conducting assessments similar to the UAS or OASIS. - Experience providing primary care to homebound persons. - Experience in care management for persons receiving home and community-based services. - Experience in home care as a paraprofessional or licensed practical nurse caring for a minimum of two patients per week. Description: The position requires conducting comprehensive assessments of patients in a home health setting. Nurses must have experience emphasizing continuity of care to reduce fragmentation and gaps in treatment. Initial in-home evaluations should include assessments of health status, strengths, care needs, and preferences. Documentation of any concerns or conflicting information during the evaluation process is essential. Nurses will discuss health care options and necessary supports with consumers. Communication between the care team members is crucial for better care coordination. Collecting quality review data to support outcome measurements is part of the responsibilities. It is important to have a comprehensive knowledge of community resources and payor requirements. Candidates must demonstrate excellent organizational, interpersonal, and communication skills.
