Registered Nurse (RN) | Home Health Location: Auburn, NY Agency: Cynet Health Pay: $2,176 per week Shift Information: Days - 5 days Contract Duration: 260 Weeks Start Date: ASAP About the Position Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 Days Shift: 5x5 Days Hours per Shift: Not Specified Experience: Two (2) years of experience in a home setting within the last ten (10) years License: Current RN Licensure in New York State Certifications: BLS (Required before start date) Must-Have: - All nurse assessors must meet the following requirements: - Two (2) years of experience in a home setting within the last ten (10) years. - Experience conducting comprehensive assessments of patients. Description: Required Licensure: Current RN Licensure in New York State. Required Experience: Two (2) years of experience in a home setting within the last ten (10) years. This includes working in the field for a certified home health agency, licensed home care service agency, or managed long-term care. Nurses must have recent experience conducting assessments that are similar to UAS, such as OASIS. Responsibilities include providing primary care to homebound individuals and managing care for persons receiving home and community-based services. Experience as a paraprofessional or licensed practical nurse caring for a minimum of 2 patients per week is essential. Preferred: Experience conducting assessments in UAS-NY, especially in the last 365 days. Experience caring for a geriatric population and managing chronic diseases in a home setting. A combination of home care experience as a paraprofessional or LPN with RN experience caring for a minimum of 2 patients per week is highly regarded. Other: Home care experience limited to one patient does not count toward required qualifications. Candidates should possess knowledge of the theory, principles, and practices of general professional nursing. Strong organizational, interpersonal, written, and verbal communication skills are essential. Candidates should be capable of communicating effectively with multi-generational consumers. Ability to work in a fast-paced, deadline-oriented environment while executing multiple complex tasks is crucial. Candidates must be willing to conduct in-field assessments. Job Responsibilities: Perform initial in-home pre-evaluations of patients, evaluating their health status, strengths, care needs, and preferences. Review medical documentation and health referral forms relevant to each case. Enter evaluation data into electronic forms and transmit as required. Document any concerns or conflicting information surfaced during the evaluation process. Emphasize continuity of care to reduce fragmentation and gaps in treatment plans. Discuss health care options and needed supports with consumers. Initiate communication for enrollment into managed care health plans. Enhance collaboration with interdisciplinary care team members for better care coordination. Collect quality review data and required documentation to support outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources and payor requirements for the target population. Meet all standards established for this position. Perform additional duties as assigned by the Regional Nurse Manager or project management.
