About the Position TravelNurseSource is working with Job.com to
find a qualified Rehab RN in Rochester, New York, 14626! Deliver
high-quality nursing care to patients in an acute rehab setting.
Assess, plan, implement, and evaluate individualized patient care
plans. Monitor neurological status and recognize changes in patient
condition. Administer medications and treatments per provider
orders. Assist with patient mobility and transfers using equipment
such as Hoyer lifts and sit-to-stand lifts. Collaborate with
physical therapy, occupational therapy, and other disciplines to
promote optimal recovery. Document all patient care and progress
accurately in Epic (preferred). Educate patients and families on
care plans, safety, and discharge goals. Maintain infection control
and safety standards in compliance with hospital policies. 5
