Sign on bonus $5,000.00We are seeking a self-motivated Substance Use Counselor to join our team! We are currently taking applications for CASAC-Ts, CASAC-P, CASACs, MHCs, LMSWs, LCSWs, and LMFTs. The ideal candidate will display strong problem-solving skills, the ability to work with diverse population, and strong documentation and organizational skills.Applicants must have a CASAC-T, CASAC, MHC, LMSW, or LCSWPosition Description:Counselor will manage a caseload of patients admitted to the Program. Counselor will perform essential counseling functions including intake, orientation, assessment, treatment planning, referrals, individual and group counseling, case management, reporting and record keeping and care coordination.Responsibilities:1. Maintain and manage a caseload2. Weekly individual and group counseling sessions (as assigned)3. Performs assessment and intake as needed4. Assist with the development of clinical treatment tracks that are diversified in language and culture that will meet the treatment and service needs of patients and families5. Assist with weekly utilization reviews based on established clinical and administrative protocols6. Assist with monthly skills-based workshops relative to chemical dependency7. Assist with the review, process and distribute incident reports based on established agency-wide protocol as directed8. Provide input for employee reviews as requested9. Assist in the preparation of weekly case conference and case reviews10. Assist with the assessment and intake process11. Assist with the clinical discharge process12. Responsible for adhering to a standard of maintaining 10 billable units of service per day based on caseload13. Record individual patient data in NYC HRA STARS as required weekly/monthly14. Monitors patient toxicology testing15.
