Job Description:
**Description**Utilizing a person-centered approach, facilitates
the personal growth and development of each individual through
advocacy of their individual rights, program implementation and
documentation, community involvement and by maintaining a
nurturing, safe, healthy living environment free from abuse and
neglect.Duties and Responsibilities:Remain alert and responsive
while on scheduled shift(s).Facilitates active participation of the
residents by implementing their individual program plan in their
home and out in their community. Completes the necessary training
to be able to work closely with fellow staff, as well as, work
one-on-one with a resident in an isolated environment within the
house or out in the community. Responsibilities include, but are
not limited to, engaging in necessary lifting, moving and
positioning tasks; facilitating improved gross motor, fine motor
and range of motion skills; promoting self care, activities of
daily living and personal hygiene; fostering receptive and
expressive communication through many different avenues such as,
sign language, pictograms, electronic devices, etc; assisting with
meals through modeling appropriate behavior, therapeutic feeding
and use of therapeutic equipment; assisting with recreational
activities and facilitating active engagement in community outings;
accurate and consistent implementation of behavior support
plans/SCIP-r.Navigates the Agency's IntrAnet, time tracking program
and the Agency's email system. Regularly checks the IntrAnet, time
card and email and is responsible for all information shared in
written and electronic form. Approves time card according to the
payroll schedule and responds to email requests in a timely
manner.Accurately and consistently completes all required
documentation practices, in a contemporaneous manner, utilizing the
PrecisionCare software and in written form, when
necessary.Facilitates and/or provides for the personal and physical
care needed by each individual including tasks such as bathing,
toileting, eating and dressing.Performs housekeeping tasks as
designated by the shift supervisor/Residence Coordinator including
tasks such as meal preparation, cleaning, laundry and shopping.In
keeping with confidentiality and HIPAA practices, communicates a
wide range of information regarding the residents to a variety of
people in and out of the residential program such as co-workers,
teachers, supervisors and parents.Is responsible for following the
Code of Ethics for Direct Support Professionals, as well as, show
proficiency in the Core Competencies to ensure person-centered
supports for the residents.Attends and actively participate in, at
least, biweekly staff meetings as scheduled or requested by the
Residence Manager.Attends and actively participates in case
reviews, in-service sessions, team meetings and other meetings upon
request.Implements fire, safety and emergency procedures and
assists with fire drills.Implements and follows all required
individual programming and Agency/Program-specific protocols and
procedures accurately, consistently and according to how they are
written.Safely drives Agency vehicles and personal vehicle, when
necessary, to assist in transportation to and from recreational
activities, community events and other appointments as
required.Maintains Agency-Approved Driver status, as well as,
certifications in Medication Administration, SCIP-r, CPR and First
Aid. Completes all required ongoing/annual trainings and
inservices, as assigned.Administers medications to residents in
accordance with the Residential Medication policies and
procedures.Assumes shift supervisory responsibilities when
requested.May serve as an advocate for one of the residents and
implements responsibilities such as writing reports, data
collection, case reviews, regular classroom/day program visits,
etc.Other duties may be assigned.**_Must maintain Agency-Approved
Driver status, certification in CPR, First Aid, SCIP-r, NYS
Medication Administration and all other training mandated by OPWDD
