Job Description:
Rochester, NY, USA Commensurate with experience. Full
TimeResponsibilitiesProvides services to individuals with
development disabilities in the Community Habilitation Program.
Apply the principles of self-direction and Person Centered
Planning; Assist individuals in exploring their community and using
community resources; Assist individuals in participating in
activities of their choice; Work with individuals with
developmental disabilities and receive training and instruction
from the individuals who are co-managing their services; Provide
all services as outlined in the Staff Action Plan; Provide support,
education and life skills training to individuals receiving
services in the program; Assist individuals in living independently
in the community; Educate individuals with developmental
disabilities on the importance of attending and following up on all
medical appointments pertaining to health and safety; Train
individuals with developmental disabilities on identifying any type
of abuse and resources to seek help if they are endangered; Assist
individuals with developmental disabilities with obtaining
alternative housing when necessary; Assist individuals with
developmental disabilities in maintaining a safe home environment;
Maintain contact with the service providers involved with all
assigned individuals with developmental disabilities; Conduct
weekly in home visits as specified in the Life Plan and Staff
Action Plans; Accurately complete monthly summaries and daily
documentation and submit by the deadline as specified by the
Community Services Department; Complete all monthly units of
service as indicated in the Life Plans per assigned caseload;
Complete and submit accurate weekly and monthly billing
documentation and all necessary paperwork as specified by OPWDD and
Lifetime Assistance. Must have a valid New York State Driver's
License, a reliable and properly insured vehicle to transport
service recipients as required.Qualifications:High School Diploma
or equivalency required. Any combination of education and
experience that demonstrates the abilities and knowledge for
successful performance will be considered. Must possess the ability
to: read, analyze and interpret policies, procedure and
regulations; write reports, business correspondence; effectively
present information and respond to questions from groups of
individuals, their families, employees, other service providers and
the general public; define problems, collect information, establish
facts and draw valid conclusions; interpret extensive instructions
and deal with a variety of complicated options. While performing
the responsibilities of the DSP Community Hab Counselor, the
employee is often required to sit and use their hands and fingers
to handle or feel and to manipulate keys and a keyboard. The
employee is occasionally required to stand, walk, and reach with
arms and hands. Vision abilities required by this job include close
vision. May be required to lift up to 35 pounds. Must successfully
complete all required training including but not limited to First
Aid/CPR, SCIP (Strategies for Crisis Intervention and Prevention)
and Medication Administration. Must be able to lift, move and
transfer up to 35 pounds. Subject to background checks including
but not limited to being fingerprinted/criminal history record
check, NYS Child Abuse Registry, OPWDD Staff Exclusion List and
Mental Health Law 16.34. Must meet LAI Vehicle Operator
