Job Description:

Position Summary: The Sliding Fee Coordinator determines patient eligibility for the Rochester Regional Health Sliding Fee Discount Program. The Sliding Fee Coordinator is responsible to track and ensure eligible patients recertify for the program annually. The Sliding Fee Coordinator may also assist with performing monthly audits, reporting and analysis, and program evaluation activities. Key Responsibilities: * Reviews work queues and reports to identify all under and uninsured patients. * Receives referrals from community agencies and internal health care team members. * Assesses patients and their families to determine their potential for the Sliding Fee Discount Program. * Reviews and processes incoming applications and documentation. * Maintains reports in order to track patients who need to complete their annual recertification paperwork and follows up when necessary. * Helps conduct program evaluation activities, which may include phone calls, mailers and surveys. * Performs monthly audits of processed applications and documentation to ensure compliance with the RRH Federally Qualified Health Center Sliding Fee Discount Program Policy. * Maintains reports, provides necessary documents and data by deadlines and upon request of management. * Documents clear and concise notes in the electronic health record (Epic) according to policies and procedures. * Understanding of the verification process through insurance websites, and a basic understanding of Patient Financial Services billing process. * Obtains an RRH release of information and other consents when necessary. * Upholds HIPAA and RRH policies and procedures. * Serves as a liaison between patients and family members, RRH Patient Financial Services, RRH Affiliates, and community agencies. * Collaborates and communicates effectively with interdisciplinary teams as needed. * Participates in hospital, affiliate and departmental committees where appropriate. * Participates in regular exchanges of information among internal team members and community agencies when appropriate. * Provides input and potential resolution to management regarding policies and/or procedures. * Demonstrates an understanding of the diversity and culture of the various populations served. * Encourages cross training with other departmental team members to enhance knowledge and skills. Fosters team building and cross coverage as necessary. Desired Attributes: * Excellent communication and organizational skills, ambitious and flexible. * Strong computer skills and program proficiency. * Bilingual preferred. Minimum Qualifications: * AAS in related field with two years of experience preferred or an equivalent combination of work experience in financial counseling, billing and/or customer service required. * Demonstrate experience working with diverse populations.
