The PC Intake and Referral program for non-R&P populations is intended to address the initial resettlement needs of Office of Refugee Resettlement (ORR) eligible individuals who did not benefit from service provision through the Reception and Placement program. The PC-IR Coordinator will conduct an intake and initial assessment that identifies each individual's primary need(s) and provide referrals to programs that will address the needs that have been identified.

The PC-IR Coordinator is responsible for all required case file documentation and case file maintenance utilizing all mandatory forms provided by the funder as well as reporting and tracking services.

Essential Duties and Responsibilities

Works in collaboration with other programs within the Refugee, Employment, Immigration Services (REIS) Department and partners.

Arranges for appropriate language (interpreting) assistance through the Language Line or in-person interpreters to provide high quality assessment and referral services, ensuring the individual seeking assistance understands the services provided and those available to them.

Confirms program eligibility as defined by ORR, securing appropriate eligibility documentation.

Conducts intake(PC-IR-02) and assessment(PC-IR-03) to address initial resettlement needs of the ORR eligible individual.

Completes a service plan that addresses the client's identified vulnerabilities to include activities provided to address and resolve needs, along with projected completion time frames within the 90-day service period.

Meets all required compliance timeframes for documentation of services as identified by the program and funder.

Assesses direct assistance need of the ORR eligible population to address insecurity related to housing, medical needs, or emergency material goods.

Ensures that the provision of any direct assistance provided is linked to an appropriate referral to address the root cause of the identified need.

Tracks the distribution of client direct assistance utilizing a tracking system, the Financial Record Form (RF-19) and supporting RF-35 Receipt of Direct Assistance.

Conducts outreach activities to inform the community, including refugee serving partners, legal immigration offices and community centers about the availability of support.

Makes referrals to appropriate community programs based on the individuals identified primary need documenting on PC-IR-06

Completes 3 identified reports the intake, initial and closing assessments, in the MRIS database.

Enters client level data into appropriate forms and databases submitting required reports defined by the funder.

Works with other RIES Dept team members to facilitate appointment times for next service steps at CCFCS.

Completes case notes reflecting all services provided on behalf of the client and the outcome on the PCIRU case note log.

Tracks and reports on intake activities on a regular basis as required by the program manager or director.

Assist with other administrative aspects of CCFCS' Refugee and Immigration Department as needed.

Education: Associate or bachelor's degree or equivalent combination of education and experience

Credentials: Valid and clean NYS Driver's License.

Experience: Experience working with volunteers, refugees, and local service providers preferred. Ability to speak an additional language to English i.e., Cuban Spanish, Haitian Creole, is preferred. Ability to complete spreadsheet and Intake, Assessment and Referral Form in English.
