Job Description Job Description Description: Summary : Under the
supervision of the HCBS Director, the Day Habilitation Coordinator
oversees the day-to-day operations of the Day Habilitation Program.
The coordinator is the introductory link to potential program
participants. Essential Functions: Reasonable Accommodations

to the Director of HCBS. Assures all emergency procedures are
implemented as required by Federal, State, Agency and Program
regulations, policies, and procedures. Schedules and conducts
monthly Day Habilitation staff meetings. Ensures Day Hab staff is
up to date on all trainings. Monthly meeting with Nurse to ensure
medical needs are met. Ensures the development of the individual’s
service planning process. Ensures Programs meet State and Federal
Regulations. Ensures billing documents are completed according to
Medicaid Standards. Communicates and Coordinates with the nursing
staff to ensure consumer's medical needs are met. Establish and
maintain good working relationships with funding sources and other
service delivery agencies. Ensure full enrollment in the Day
Habilitation by recruitment of new program participants and
processing of required paperwork. Meets with Community Resources to
secure volunteer opportunities for Day Habilitation participants.
Connects individuals with community resource for volunteers,
according to the participant skill level, strengths, and desire.
Does Intake and Pre-admission assessment in conjunction with the
intake committee. Organizes and implements activities, which will
facilitate involvement of employer, potential program participants,
and other DD providers. Ensures the provision of all service plans
for participants. Will conduct fire drills as per regulation. Stays
abreast with the latest fire safety practices. Submit monthly
report, and quarterly written reports pertaining to program
participant and program progress. Responsible for submitting work
orders as needed. Attends trainings and meetings assigned by
supervisor. Recruits, hires, and supervises all Day Habilitation
Staff Any other duties as assigned by supervisor. Authority: This
position holds a relatively high degree of independence within the
framework of the agency's policies and procedures. The individual
must be resourceful and able to display initiative and
understanding of management systems and interpersonal relations.
Must keep immediate supervisor abreast of issues and concerns which
could constitute intervention from higher level management.

related field. Experience working with OPWDD regulations and
policies. Experience working with individuals with developmental
disabilities. Bilingual Spanish and English is require. Must
possess a valid NYS driver's license and meet Ibero's Vehicle
Operator Requirements. A combination of training and experience
other than the specified, if judged to be adequate for the job, may
be accepted by the President and CEO Physical Demands: The position
does require occasional standing, squatting, lifting of up to
approximately 30 lbs. And frequent sitting. Ibero American Action
