Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Start Date: ASAP - Duration: 13 weeks - 40 hours per week - Shift: 8 hours - Employment Type: Travel Title: Physical Therapist – Outpatient Location: Syracuse, NY Duration: 13 weeks Shift: Days, Monday–Friday 08:00 – 16:30; one weekend per month Job Responsibilities and All Required and Preferred Skills and Requirements Role Overview /t - /t Evaluate patients, develop and implement individualized physical therapy treatment plans in the outpatient setting. /t /t - /t Establish specific, measurable goals in collaboration with patients and interdisciplinary healthcare teams. /t /t - /t Provide direct patient care, maintain timely documentation including progress notes and discharge summaries. /t /t - /t Attend clinics, rounds, and conferences and share relevant patient information with care teams. /t /t - /t Participate in staff education programs and training of new staff. /t /t - /t Serve as clinical instructor for senior-level physical therapy students and support new program development initiatives. /t Required Qualifications /t - /t Education: Bachelor's degree in Physical Therapy. /t /t - /t Licensure: New York State Physical Therapy license or eligibility. /t /t - /t Experience: Minimum 2 years of acute and/or outpatient physical therapy experience. /t /t - /t Must successfully pass pre-employment health clearance. /t HonorVet Technologies Job ID 31691733.
