Registered Nurse (RN) | Home Health Location: Canandaigua, NY Agency: Cynet Health Pay: $2,161 per week Shift Information: 5 days x 7 hours Contract Duration: 260 Weeks Start Date: ASAP About the Position Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 Days Shift: Day Hours per Shift: 7.5 Experience: Two (2) years of experience in a home setting within the last fifteen (15) years. License: Current RN Licensure in New York State Certifications: BLS required before start date Must-Have: Recent experience conducting comprehensive assessments of patients Description: Required licensure includes current RN licensure in New York State. Candidates must have two years of experience in a home setting within the last fifteen years. Nurses must have recent experience conducting comprehensive assessments of patients. Ability to review health status questions and make evaluations and determinations based on criteria for enrollment. Ability to work with technology in the workplace. Knowledge of the theory, principles, and practices of professional nursing. Understanding of laws and regulations governing nursing practices. Excellent organizational, interpersonal, written, and verbal communication skills. Ability to communicate and work effectively with a diverse consumer base. Demonstrated sensitivity to the concerns of others. Ability to perform comfortably in a fast-paced, deadline-oriented work environment. Ability to successfully execute many complex tasks simultaneously. Strong ability to work independently. Responsibilities include performing initial in-home pre-evaluations of patients. Evaluating health status, strengths, care needs, and preferences. Guiding the development of individualized long-term care service plans. Reviewing consumer medical documentation and health referral forms as relevant. Entering evaluation data into electronic forms and transmitting as required. Documenting any concerns, conflicting information, and issues that surface during the evaluation process. Emphasizing continuity of care to reduce or eliminate fragmentation, duplication, and gaps in treatment plans. Discussing health care options, supports needed, and service vendor options with consumers. Initiating communication between the Call Center and eligible consumers. Enhancing communication and collaborative relationships with interdisciplinary care team members. Collecting quality review data and required documentation to support outcome measurements. Maintaining knowledge of community resources and payor requirements for the target population. Meeting all standards established for this position as outlined in the corresponding annual performance criteria.
