Growing together. /n /n In this RN Clinical Coordinator role, you
will be an essential element of an Integrated Care Model by
relaying the pertinent information about the member needs and
advocating for the best possible care available, and ensuring they
have the right services to meet their needs. /n /n There will be
travel expectations throughout these counties and region. /n /n If
you reside in the state of New York, you’ll enjoy the flexibility
to work remotely * as you take on some tough challenges. /n /n
Primary Responsibilities: /n /n - Assess, plan and implement care
strategies that are individualized by patient and directed toward
the most appropriate, lease restrictive level of care /n - Perform
the NYS UAS Assessment in the member’s home at least twice per year
and as needed /n - Identify and initiate referrals for social
service programs, including financial, psychosocial, community and
state supportive services /n - Manage the care plan throughout the
continuum of care as a single point of contact /n - Communicate
with all stakeholders the required health-related information to
ensure quality coordinated care and services are provided
expeditiously to all members /n - Advocate for patients and
families as needed to ensure the patient’s needs and choices are
fully represented and supported by the health care team /n /n /n

Qualifications: /n /n - Current, unrestricted RN license for the
state of New York /n - 2 years of relevant clinical work experience
/n - 1 years of experience of community case management experience
coordinating care for individuals with complex needs /n -
Experience in long-term care, home health, hospice, public health
or assisted living /n - Proficiency with MS Word, Excel and Outlook
/n - New York state issued ID or ability to obtain one prior to
hire /n - Ability to travel in assigned region to visit Medicaid
members in their homes and/or other settings, including community
centers, hospitals or providers' offices /n /n /n Preferred
Qualifications: /n /n - Behavioral health or clinical degree /n -
Experience with electronic charting /n - Experience with arranging
community resources /n - Field based work experience /n -
Background in managing populations with complex medical or
behavioral needs /n - Proficient in use of UASNY /n /n /n The
hourly range for this role is $28.61 to $56.06 per hour based on
full-time employment. Pay is based on several factors including but
not limited to local labor markets, education, work experience,
