- SUMMARYAs a Hospital Hospice Liaison your superior assessment
ability and demonstrated compassion allows for the seamless
transfer of patients from the facility to hospice services. The
Hospital Hospice Liaison's primary goal is to facilitate the
patient discharge from the hospital setting to hospice services in
the community and/or at Hildebrandt Hospice Center and work
one-on-one with families/caretakers on patient's next steps in
their terminal journey. The Hospital Hospice Liaison will provide
superior customer service by modeling the Brand Promise and Core

CENTERHildebrandt Hospice Care Center is an--11-bed facility at
2652 Ridgeway--Avenue that embodies--the--promise--of comfort
and--dignity by providing end--of--life care. Nurses, Evaluators
and Hospice Aides work directly with patients and support their
families through the loss of a loved one. We partner with hospitals
and health facilities to provide end to end continuity of patient
care. We are a responsive, collaborative partner in advocating for,
and providing access to effective, innovative, culturally competent
and cost-efficient services for the communities we serve.
Hildebrandt Hospice Care Center is a part of the broader Hospice

tour: !ATTRIBUTES
- Five (5) years of RN experience
- BSN from an approved program or an ASN
- Experience in working in partnership with the interdisciplinary
healthcare team--
- Excellent interpersonal written and verbal communication
skills
- Experience utilizing an Electronic Medical Record and Microsoft
office
- Demonstrated ability to apply decisive, sound, objective
clinical judgment for cases with varying complexityRESPONSIBILITIES
- Evaluation & Collaboration. Evaluate eligibility, needs and
care resources for patient; obtain insurance information and
authorizations for appropriate reimbursement; collaborate with
facility care coordinators, social workers and physicians to find
solutions for challenging patients
- Assessment & Development. Complete a comprehensive clinical
assessment for hospice services and develop a home care plan;
coordinate all services, including professional, paraprofessional
services, durable medical supplies and equipment--
- Documentation. Complete all required documentation in patient's
facility record; gather and analyze data and complete all assigned
report EDUCATION AS: Nursing (Required) LICENSES/CERTIFICATIONS RN
- Registered Nurse - New York State Education Department (NYSED)
