702506 - Home Infusion Clinical Nurse - Rochester, NY - 13 -Week
Contract
Sigma healthcare team is currently looking for a Home Infusion
Clinical Nurse to work onsite in Rochester.
Shift: D/E/N ROTATING SHIFT, -Flexibility of Day shift
8:00am-4:30pm or Evening shift 11:00am-7:30pm, weekends per
need.
Job Requirements:
- Performs initial assessment and ongoing reassessments for each
patient.
- Monitors and maintains infusion sites and systems.
- Administers and monitors antineoplastic agents, parenteral
nutrition, investigational medications, blood and/or blood
components and pain management medications.
- Follows the seven (7) medication rights and reduces the
potential for medication errors.
- Performs venous and arterial punctures and
phlebotomies.
- Assesses patient's condition, initiates plan of care,
re-evaluates and updates as necessary.
- Observes and assesses patients for adverse reactions and/or
complications to parenteral therapy, antineoplastic agents,
medications and blood and/or blood components.
- Ability to adequately assess and reassess pain. Utilizes
appropriate pain management techniques.
- Educates the patient and family regarding pain
management.
- Performs all aspects of patient care in an environment that
optimizes patient safety and reduces the likelihood of
medical/health care errors.
- Supports and maintains a culture of safety and
quality.
- Obtains all necessary physician orders and collaborates with
physician as patient's needs dictate.
- Re-evaluates the need for continued care on an ongoing basis;
initiates recertification as necessary.

Job Qualifications:
- Current state licensure as a Registered Nurse.
- Experience as an RN commensurate with one (1) of the
following:
One (1) year acute care experience within the last two (2)
years
One (1) year community health or public health experience within
the last two (2) years
One (1) year home healthcare experience within the last two (2)
years
One (1) year outpatient facility experience within the last two (2)
years
- Current certification as CRNI preferred.
- Current BCLS certification.
- Current health certificate/physical examination and TB
testing.
- Successful completion of Background and Drug
screening.
- Knowledgeable in the use of infusion equipment.
- Valid state driver's license and reliable automobile.
- Proof of current automobile insurance.
- Able to communicate effectively in English, both verbally and
in writing
- Basic computer knowledge
- Must have effective communication skills.
- Demonstrated leadership capabilities with the ability relate
well to staff and other professionals.
