Responsibilities Position Summary: The Sr. Psychiatric Targeted
Case Manager is an integral member of the Strong Ties Clinic Team,
providing comprehensive case management for individuals who have
serious and persistent mental illness and have not connected with
traditional mental health outpatient clinics or Health Home Care
Management. The ST Clinic Psychiatric Targeted Case Manager works
with clients to develop client-driven goals and creative ways to
help clients meet these goals. The Sr. Psychiatric Intensive Case
Manager provides cross training to other team members and helps
ensure that all team members are aware of and focused on helping
clients work on their recovery goals. All team members receive
training in teaching the OMH wellness management and recovery
program to clients. The Psychiatric Intensive Case Manager
subscribes to the Department's goal of creating a culturally
competent environment by treating patients/families, trainees,
research subjects and coworkers in a sensitive and caring manner
with appropriate attention to cultural differences.
Responsibilities: Direct Clinical/Patient Care + The Psychiatric
Intensive Case Manager assists clients in maintaining themselves
successfully in the community. These tasks include, but are not
limited to, assisting clients in finding and furnishing apartments,
shopping, setting up financial budgets, getting entitlements,
transporting to appointments, and learning skills of daily living.
+ The Psychiatric Intensive Case Manager also participates in an on
-call rotation, with other ACT team members, to cover the emergency
ACT phone approximately once every seven weeks or as needed to
support patient care. The ACT model calls for clients to have 24
hour direct access to ACT support in case of emergencies. +
Psychiatric Intensive Case Manager provides feedback to the
treatment team regarding patient progress in recovery, and
facilitates and supervises the placement of patients in other
community programs as necessary for the client-specific family
goals and/or needs. + The Psychiatric Intensive Case Manager serves
as liaison between the ACT program and other community agencies
that provide patient/family support services and resources as
indicated. Indirect Clinical/Patient Care + The Psychiatric
Intensive Case Manager is responsible for coordination and primary
implementation of case management activities. + The Psychiatric
Intensive Case Manager consults with the ACT Team Leader and
Psychiatrist regarding response to treatment interventions. + The
Psychiatric Intensive Case Manager participates in daily morning
meetings, weekly treatment team meetings, and weekly supervision
with ACT Team Leader. + The Psychiatric Intensive Case Manager
provides program consultation and education to community members. +
The Psychiatric Intensive Case Manager documents recipient progress
as per established guidelines. + The Psychiatric Intensive Case
Manager will perform other tasks as assigned by the ACT Team
Leader. Qualifications: Associate's degree in health-related field,
previous experience in human services (preferably in a psychiatric
setting); and/or an equivalent combination of education and
experience. Must possess a valid New York State Driver's License
and have access to automobile. Preferred Qualifications Master's
degree preferred and experience working with the SPMI population.
Licensure issued by the New York State Department of Education in
