STATUS: Part Time - 25 Hours LOCATION: MS Achievement Center DEPARTMENT: Nursing Support SCHEDULE: As scheduled by department ATTRIBUTES - One (1) year of experience as a geriatric certified nursing assistant in home, community based or long-term care (LTC) setting required - Compassionate, warm and patient focused - Excellent communication and interpersonal abilities - Valid, current NYS Nursing Assistant certification (CNA) required. RESPONSIBILITIES - Patient Care.Perform direct care (toileting, feeding, grooming, ambulation, transfer, etc.) as determined by care plan; deliver therapeutic care established by physical and occupational therapists, individually and in group settings; prepare and distribute meals and snacks and assist in clean up - Collaboration & Documentation.Participate in the planning and implementation of registrant care through regular team meetings and care conferences; document (Care Taker or Flow Sheets) care and therapy on a daily basis; observe and report changes in participant's status - Recreational Management.Assist with recreation activities and leisure programs on an individual and group basis; supervise and care for participants when on outings Rochester Regional Health System is an Equal Opportunity / Affirmative Action Employer.
