Job Description:

The Medical-Surgical Clinical Coordinator is responsible for the provision of comprehensive clinical pharmacy services for the adult internal medicine patients in addition to directly supervising clinical activities on the medical-surgical team. Clinical Pharmacy Specialists on the team directly report to this position and this position has “dotted line” oversight of clinical activities performed by clinical staff pharmacists. The incumbent in this position is to optimize safe, effective and cost-effective drug therapy. Activities include participation as a member of an inter-disciplinary patient care team(s), provision of pharmacotherapeutic and pharmacokinetic consults, patient and provider education, and the development of best practice guidelines and policies related to pharmacotherapy. The Clinical Coordinator trains pharmacy learners, serves as an expert in their specialty area of practice, innovates pharmacy practice, optimizes the medication use system, both leads and participates in department initiatives, and delegates tasks and responsibilities where appropriate. The incumbent in this position leads clinical initiatives, clearly communicates, and collaborates with numerous disciplines across the organization. The Clinical Coordinator will also have a working knowledge of the operational aspects of the department and contribute to the overall objectives of the department and prepare, compound, and dispense prescriptions and medications for patients of SMH (Neonate, Pediatric, Adult, Geriatric) in accordance with all applicable state and federal laws and regulations when required. Typical Duties: Clinical Pharmacy Practice:

  • Provide comprehensive clinical pharmacy services for a specialized area of pharmacy practice including:
    • Participation on inter-disciplinary patient care team rounds as appropriate to the specialty
    • Consultative input on safe and effective drug therapy
    • Daily management of established clinical pharmacy programs or services
    • Monitors assigned patient populations for clinical progress including renal/hepatic function, culture results, and other applicable laboratory tests. Recommends adjustments to therapy as necessary.
    • Patient education and discharge counseling
    • Authorizing/verifying orders
    • Supervising drug distribution when appropriate and required
    • Document clinical pharmacy activities as appropriate in the medical record or other systems (e.g. event reporting system).
    • Work collaboratively with other clinicians and health-care providers to implement and maintain innovative disease or medication management programs and clinical pharmacy services
    • Participate in the development of global medication use management programs within the University of Rochester Medical Center including clinical guidelines, critical pathways, disease management and drug use programs
    • Provide clinical leadership in area of specialty by serving as a mentor, coach and role model for all pharmacists in the department. Recognize the need for mentoring and education and reach out in a positive and constructive way to facilitate learning and professional growth.
    • Direct and supervise the work of technicians to ensure appropriate workflow as required.
    • Verbal communication to all hospital staff is conducted in an effective and professional manner.
    • Conduct of QA, MUE, or other research to document the value and contribution of clinical pharmacy services at URMC within area of specialization.
    • Participates in departmental Q/A processes such as reporting and following up on medication incidents, reconciling MARs, and cost-savings initiatives etc. Training:
      • Maintains a personal and professional commitment to the training of future generations of pharmacists as exemplified by:
        • Serving as a preceptor for pharmacy residency training programs offered at the University of Rochester Medical Center
        • Contributing to student training experiences for IPPE and APPE rotations as assigned
        • Providing didactic lectures, case studies and other interactive teaching programs to pharmacy students, pharmacy residents, and pharmacists.
        • Provides didactic lectures to various divisions and departments of the medical center as requested including nursing, medical/surgical residents and students training in allied health professions Problem Solving :
          • Thoroughly resolves problems with appropriate and direct communication to all pertinent staff in a clear and concise manner.
          • Proactive in identifying patient care problems and does not pass on problems to the next shift without exhausting reasonable attempts at resolution and communicating progress.
          • Problem solving will often go beyond patient-specific issues, and may require participation on interdisciplinary teams or revising hospital or departmental policy, procedures or treatment guidelines.
          • Contacts supervisor for assistance when solution is beyond his/her expertise or authority. Service:
            • Active membership and participation in local, regional and national professional organizations in clinical pharmacy and/or specialty areas of practice.
            • Presentation of clinical education or research results at local, regional and/or national meetings.
            • Provision of continuing professional education to peers in the community, region, and/or nationally. Recognition of expertise in specialty area of practice at a local and national level.
            • Service on departmental, hospital or medical center committees or task forces. Management Responsibilities :
              • Schedule management. Responsible for establishing the scheduling of staff in relevant area of supervision in coordination with other managers and Associate Director to provide adequate coverage to meet workload demands. Overtime and extra compensation will be kept to a minimum, and justification provided on a monthly basis as requested by the Associate Director or Director.
              • Recruitment and Retention. Participates in the interview and hiring decisions for staff members assigned to the relevant area of supervision. Also is responsible for identifying measures to maintain or improve employee satisfaction, reward and recognition to promote employee retention.
              • Performance evaluation. Responsible for the completion and presentation of annual and on-going evaluations of performance of employees in relevant area of supervision. Performance problems are addressed proactively, action plans established with employee, and follow-up evaluation and documentation completed as scheduled. Directly supervise clinical specialists and/or clinical pharmacy technicians on the team and contribute to professional development and performance evaluation of all pharmacists on the team.
              • Conflict Resolution. Conflict is handled in a direct, timely and professional manner. Disciplinary action, when required, is handled confidentially and is well documented with an action plan for correction when appropriate.
              • Interdepartmental management. The Clinical Coordinator will be regularly and routinely involved in interdepartmental meetings, projects and policy making. This is necessary to successfully conduct the business of the functional unit under their supervision and management. Their Associate Director should be aware of all ongoing interdepartmental planning and activities, but may not be directly involved.
              • The Clinical Coordinator will need to successfully delegate tasks to those under their supervision. The manager will need to establish a plan and expectations for delegated duties, and follow-up to make sure they are being completed according to the plan.
              • Clinical workload and outcome statistics. The Clinical Coordinator will be responsible for tracking and reporting statistics related to clinical programs which focus on value and outcomes to their Associate Director on an established schedule. The statistics/data to be tracked and the frequency of reporting will be agreed upon with the Associate Director.
              • Budgeting and Performance Improvement. As appropriate, the Clinical Coordinator will be responsible for budget performance improvement goals and projects at the local/team level and will track and report this performance to their Associate Director.
              • Advance pharmacy department initiatives. Propose opportunities for improvement and initiatives to pursue to Associate Director(s). Manage and/or lead initiatives identified by departmental leadership as appropriate. Partner with other pharmacy department teams to ensure safe, efficient processes.
