Our employees are engaged and perform meaningful work, everyday.The Nurse Case Manager is responsible for assessing, planning, implementing, and coordinating all case management activities with residents to evaluate the medical needs and to facilitate the overall health and wellness of the residents.Qualifications:--- A valid NYS RN license in good standing--- Prior nursing experience with an adult population, providing supportive care services in an adult care facility or long-term care building.
