To manage the overall operation of the Medical / Psychiatric transitional residence and in collaboration with Site Supervisor / Program Director, provide supervision to all staff, the oversight of the quality of care provided to the residents. Work with SS (Site Supervisor) and PD (Program Director) to coordinate and arrange for the ultimate placement of individuals to safe and more permanent / appropriate housing within 90 days of admission.

Responsibilities

1. Responsible for monitoring resident care to include the coordination of the resident's health care needs

with all relevant service providers.

2. In collaboration with SS and PD, responsible to hire, supervise and schedule residential staff.

3. Observe, monitor and document any change in a resident's psychiatric condition and promptly report to supervisor and

primary relevant health care provider.

4. Encourage, prompt and remind as needed to support residents' medications are taken as prescribed. Depending on

each person's abilities, this may include requesting a home care referral or assistance from health care provider.

5. Make or follow up on referrals to health care providers to ensure all appropriate treatment / services are in place for

each resident based on their individual needs.

6. Oversee and ensure preparation and set up of 3 nutritious meals per day, including the oversight of

supplies and / or groceries for the site.

7. Work with each resident on a formal discharge plan within 48 hours of admission to the program.

8. Coordinate all psychiatric, medical, legal, funding resource and other necessary appointments and arrange

transportation as necessary. Transportation provided in own personal vehicle at times - when needed.

9. Actively engage in on-going communication with healthcare providers to provide status updates on residents' health

status and conditions as requested by supervisor.

10. Responsible for securing more permanent / appropriate housing for each resident within 90 days of admission. This

will often require helping secure funding for each person, including taking them to DSS or SS, helping complete

funding and / or housing application, following up with phone or email contacts.

11. In collaboration with SS and PD, is responsible for the overall upkeep of the residence, including housekeeping,

maintenance and repairs and exterior grounds; monitoring and ordering necessary supplies.

12. Responsible for the oversight of the laundering of clothing, sheets, bedding etc. as needed.

13. Perform any other duties necessary for the effective and efficient operation of the residence.

ADMINISTRATIVE RESPONSIBILITIES:

1. Maintain a basic health record for each resident which includes contact information, medical information, and the

documentation of all contacts with healthcare providers.

2. Oversee and review all financial transactions including budgets, staff payroll and site expenditures.

3. Responsible to ensure staff receive and maintain all necessary and relevant trainings as it relates to this program.

4. Ensure all residents are treated with dignity and respect at all times.

5. Maintain resident confidentiality by ensuring compliance with all HIPPA regulations.

6. Coordinate and assume on-call responsibilities for the program.

7. Follow all health and safety regulations for the betterment of the staff and residents.

8. Perform any other duties necessary for the effective and efficient operation of the residence.

Qualifications

QUALIFICATIONS:

2-year degree in health-related services and at least one year of experience working with persons with medical conditions and / or mental illness / SPMI (serious and persistent mental illness) and supervisory experience or a combination of education and experience in managing a residential program.

A good working knowledge of the residential services available to people in need of specialized housing is preferred. Must have a current valid NYS driver's license that meets agency's clean driving record policy. The use of one's personal vehicle is also a requirement.
